If you're managing multiple medications — especially if you have Medicare Part D or certain other insurance plans — there's a good chance you're eligible for something called Medication Therapy Management, or MTM. And here's the part that surprises most people: it's often completely free.
I've been doing MTM consultations at Vine Discount Pharmacy for years, and I'm always amazed by how few patients know this service exists. So let's talk about what it actually is, who qualifies, and why it's worth your time.
What MTM Actually Means
Medication Therapy Management is essentially a comprehensive review of everything you're taking — prescriptions, over-the-counter medications, vitamins, supplements, even things like CBD oil or herbal teas. We sit down together and look at:
- Whether your medications are working as they should
- Possible interactions between drugs
- Duplicate therapies (it happens more than you'd think)
- Whether there are more affordable alternatives
- Side effects you might be experiencing
- How to take everything properly — timing, food interactions, all of it
Who Pays for This?
Here's the beautiful part: Medicare Part D plans are required to offer MTM to certain beneficiaries at no cost. Many private insurance plans cover it too.
You typically qualify if you:
- Take multiple medications for chronic conditions (the threshold varies by plan, but often it's 2–3 chronic conditions and 5+ medications)
- Have Medicare Part D
- Meet certain drug cost thresholds in a year
Why Bother? Real Benefits
I'll be honest: I've caught some concerning things during MTM sessions. A patient taking two different blood pressure medications that essentially do the same thing. Someone whose afternoon fatigue turned out to be linked to when they were taking a morning medication. Drug interactions that nobody had noticed because the prescriptions came from different doctors.
MTM can also save you real money. We might find a therapeutic alternative that works just as well but costs $40 instead of $200. Or identify a medication you've been taking for years that you no longer actually need.

What Actually Happens During a Session
When you come in for MTM (we do them right here at the pharmacy, usually in a quieter moment of the day), bring:
- All your prescription bottles
- Any over-the-counter medications you take regularly
- Vitamins and supplements
- Your insurance card
- A list of your doctors and their contact info
You leave with a written plan, and we follow up to make sure everything's working.
